Patient Forms

Please prepare for your first appointment by completing our new patient information forms. Download the forms to your computer to complete at home and then bring them with you to your first appointment. The first form requires you to fill out medical history and general information. It is important to have a good understanding of overall health before you undergo orthodontic treatment. The Authorization for Release of Information form indicates who we can discuss your treatment with and how we can communicate with you about your treatment. Please read our offices Notice of Privacy Policy which is attached to these forms and sign the Acknowledgement of Receipt of Privacy Practice form acknowledging that you have read it.
